CAMP BUCKO / CONTACT / CONSENT FORM
We at Camp BUCKO would like to introduce you to our Camp BUCKO program as your child has experienced a burn injury.
Although your child is not yet of age to attend our residential week of camp and/or our Family Camp we wish to maintain communication with you and your family at this time.
Your child will be eligible for our residential camp week once he/she is 7 years of age.
Your child and his/her immediate family will be eligible to attend our Family Camp once your child is 5 years of age.
Camp BUCKO would like to maintain contact with you and your family until this time.
Completing and submitting the CONSENT TO CONTACT form below; will allow Camp BUCKO to maintain ongoing communication.
